PERSONAL DATA PROCESSING INFORMATION & CONSENT DECLARATION

I, the undersigned (hereinafter referred to as the “Data Subject”), hereby acknowledge that I have been fully informed about the collection, processing, and storage of my personal data. In accordance with Articles 7 and 9 of the General Data Protection Regulation (GDPR) (EU) 2016/679, and the relevant national legislation implementing it, I explicitly grant my consent to:

(hereinafter referred to as the “Company”),
to collect, process, and store my personal data for the purposes described below.

1. Purposes of Processing

My personal data will primarily be processed for Medical Record Keeping as required under Article 14 of the Code of Medical Ethics, including:

  • Recording and maintaining my medical history, examinations, and other relevant information;

  • Diagnosis and treatment related to my health;

  • Compliance with obligations arising from applicable legal and regulatory frameworks;

  • Research related to previous or future medical care provided to me.

2. Sources of Personal Data

The Company will collect personal data provided directly by me through:

  • Patient registration forms, medical history forms, and accompanying documentation;

  • Any future information communicated orally, in writing, or electronically to the Company, its employees, or associated medical professionals.

Additionally, where necessary for the above purposes, the Company may collect data from authorized third parties such as laboratories, diagnostic centers, and affiliated physicians.

3. Categories of Personal Data Processed

The data processed by the Company may include:

  • Identification Data: Full name, date of birth, ID/passport number, social security number, tax identification number;

  • Contact Data: Address, email, phone/fax numbers;

  • Payment Data: Bank account details, debit/credit card information;

  • Medical Data: Symptoms, allergies, reason for visit, diagnosis, treatment details, clinical and laboratory results.

4. Data Recipients

My personal data may be shared with:

  • Other medical laboratories or diagnostic centers for further examination;

  • Company headquarters or affiliated EU-based companies within the same corporate group (for statistical and operational purposes, excluding sensitive data categories);

  • Public authorities, judicial bodies, and legal advisors, both within and outside the EU, where legally required.

5. Data Retention Period

My personal data will be stored and processed for a maximum of twenty (20) years from the date of my last visit to the clinic.

6. Data Subject Rights

I have been informed of my rights under the GDPR and applicable national law, which include:

  • Right of Access: To obtain a copy of my personal data processed by the Company;

  • Right of Rectification: To correct inaccurate or incomplete personal data;

  • Right of Erasure: To request deletion of my personal data where processing is no longer necessary or legally required;

  • Right to Restrict Processing: To request processing limitations where data accuracy is contested;

  • Right to Data Portability: To receive my data in a structured, commonly used, machine-readable format.

Requests to exercise these rights can be submitted in writing and free of charge to info@hellenicaesthetics.com. I may also contact the Hellenic Data Protection Authority (1-3 Kifissias Ave., P.C. 11523 or www.dpa.gr) if I believe my rights have been violated.

Important Note: Withdrawing consent for essential processing may render the Company unable to provide or continue certain services. The Company retains the right to maintain minimal records to protect its legal interests.

The Company will respond to all requests within 30 calendar days of receipt, informing me in writing of the outcome or the reasons preventing compliance.

7. Processing for Marketing & Service Information

I have been informed that my personal data may also be used for:

  • Targeted marketing campaigns;

  • Delivery of newsletters and promotional material;

  • Surveys on service quality;

  • Promotion of products and services of the Company and/or its affiliated Group companies.

Such data may be shared with trusted third-party partners engaged in survey and promotional activities.

I may object to such processing at any time by sending a request to info@hellenicaesthetics.com. The Company also respects my right to control the use of photos or videos taken during treatment in future marketing activities.
